PATH is a federally funded program that provides outreach and case management for homeless adults and families, as well as those at risk of homelessness. Homelessness is defined as living on the streets, in a car or encampment, shelter, transitional housing, couch-surfing or staying with extended family/friends.
PATH specifically aims to support homeless men and women who have serious mental illnesses, co-occurring substance use disorders, and/or physical barriers. These individuals often face significant challenges in accessing services and are frequently high utilizers of emergency services.
PATH evaluates each individual’s needs and barriers to housing, develops personalized service plans, and assists with applying for and obtaining benefits. The PATH team facilitates access to essential services such as food, shelter, and hygiene, and connects individuals to primary health care, dental, mental health, and chemical dependency services. PATH also links individuals to transportation options, vocational training, and other services. For those at risk of losing their housing, PATH connects them with emergency service providers.
