Transition Support Program (TSP)

The SOUND Mobile Rapid Response Crisis Teams are specially equipped two-person units, traveling in vans, staffed by behavioral health professionals and certified peer counselors. They are designed to de-escalate mental health and substance use crises, provide immediate support, and connect individuals to long-term care.

TSP supports individuals who have been involuntarily detained in King County hospitals or Evaluation and Treatment (E&T) facilities, often after a new diagnosis, as they transition back into the community. This 30- to 90-day program focuses on your immediate needs and helps connect you to the services and resources that support your health, independence, and wellbeing.

Who These Services are For

TSP serves adults 18 and older who are referred by designated county providers. This voluntary program is designed for people who are unfamiliar with existing services and are not currently enrolled with a publicly funded agency. We welcome individuals of all races, genders, religions, and sexual orientations who are navigating mental-health challenges, substance use, or trauma, and who want to improve their wellness, live self-directed lives, and reach their full potential.

Connecting to Services: We help you access the behavioral-health care, primary care, and community supports that meet your unique needs.

Collaborative Discharge Planning: We work with you to plan a safe and successful discharge, while supporting self-management of medical and behavioral-health conditions.

Person-Centered Care: We focus on your individual goals and use culturally relevant approaches that honor your experiences.

Evidence-Based Transition Support: Our team provides coaching, education, and resources to help you build skills for self-management, advocacy, and shared decision-making about your care.

Goals

TSP’s goal is to support your safe and successful return to home or community-based settings. We work to reduce unnecessary hospitalizations, crisis responses, or other high-risk situations. Our services are recovery-oriented and focused on helping you and your family build stability, personal growth, resilience, and the skills needed for long-term self-direction. We approach every individual with empathy, helping you feel safe, seen, heard, validated, and empowered.

Locations

TSP operates from SOUND’s Capitol Hill site, with clinicians who are mobile and meet clients wherever they are, whether in the hospital before discharge or in the community afterward.

A Certified Community Behavioral Health Clinic (CCBHC) is an integrated model of care designed to ensure access to coordinated and comprehensive behavioral healthcare for everyone, regardless of ability to pay, residence, age or diagnosis. CCBHCs continue to close the treatment gap that leaves millions of people in the US unable to access lifesaving mental health and substance use care.
